When service requests are enabled, your new clients fill out a short form before their first appointment. They describe their needs, their preferences and how they plan to pay. Your team then receives the request and matches the client with the right professional.
Here's what your clients will see, step by step.
1. Starting the request
Clients can fill out the service request from three places:
the GOrendezvous search page;
your GOrendezvous home page;
the Book an appointment button on your website.
For example, from the search page, the client opens a professional's profile and clicks New client. The Service request window opens.
2. Who is the request for?
The client chooses who the request is for: Me, My couple, My family or My child or another person.
A progress bar at the top shows them where they are in the form.
3. Their information
The client enters their first name, last name, date of birth, email and preferred language of communication. They must also provide at least one phone number, mobile or home.
4. Reason for consultation
The client checks what brings them to consult. A search bar helps them quickly find a reason in the list. They can then briefly describe their situation and say whether they've consulted for mental health before.
5. Their preferences
The client indicates their availability (morning, afternoon, evening), their preferred consultation method and the location that suits them. If they wish, they can also state a gender preference for the professional.
6. Payment method
The client indicates how they plan to pay: Private insurance, Employee Assistance Program (EAP), Self-pay (no insurance) or I'm not sure yet. If they choose private insurance, they can enter the name of their insurer.
7. Quick mental health screening
The client answers 4 short questions about the last 2 weeks. Each question uses a scale from 0 (Not at all) to 3 (Nearly every day).
8. Consent and submission
The client reads the privacy notice and checks the consent box. They can add a message for your team, then click Send my request.
A confirmation message lets them know their request was received and that a team member will contact them shortly.
Good to know: Clients only see the sections you've turned on. For example, if you haven't checked the quick mental health screening, that step won't appear in their form.
