Manually Send an Intake Form

If you are in the Essential or Premium plan 👇

Follow these steps to manually send an intake form from the client’s appointment:

1. Click on the client's appointment in the schedule

2. Click on Actions tab

3. Click on Send Intake Form

4. Choose a form from the drop-down box

5. Click on Send


If you are in an older plan (Professional or Business) 👇

Follow these steps to send an Intake Form manually:

1. Click on Charting

2. Go to the Charting tab

3. Click on Files

4. Choose a client name from the list or by searching it in the Choose a client drop-down box

5. Click on Send Intake Form

6. Choose an Intake form and the professional for whom you are sending it

7. Click on Send
